Our Services

Indiana’s Home and Community based services programs allow Individuals to stay in their homes instead of moving to institutions like nursing facilities. These programs foster independence and safety, empowering people to live in the least restrictive environments possible while receiving essential support.

At IPMG, we specialize in case management and wraparound facilitation for various Indiana Waivers, including the Community Integration and Habilitation Waiver (CIH), Family Supports Waiver (FS), Traumatic Brain Injury Waiver (TBI), and the Health and Wellness Waiver (H&W).

We also provide support through the Child Mental Health Wraparound program (CMHW), which focuses on enriching the lives of children facing emotional and/or behavioral challenges by engaging families and nurturing meaningful connections within the community.

Health and Wellness and Traumatic Brain Injury Waivers

In 2024, Indiana’s Medicaid Waiver Program introduced amendments for the new PathWays for Aging and Health and Wellness (H&W) waivers–formerly combined in the Aged and Disabled (A&D) waiver–which will now serve Individuals aged 59 and younger who had received services through the Aged and Disabled waiver.

At IPMG, our case managers are dedicated to making sure that Individuals receiving the Health and Wellness or Traumatic Brain Injury Waivers have the tools and support needed to thrive, utilizing all available waiver services and supports. They provide valuable education about waiver services and assist in creating personalized support plans focused on your goals.

Family Support and Community Integration and Habilitation Waiver

Our dedicated case managers empower Individuals utilizing the Family Supports and Community Integration and Habilitation Waivers by providing essential information and tools to maximize available waiver services and supports. They guide Individuals in setting and achieving their goals while coordinating both waiver and non-waiver services, including medical, social, educational, and community support.

Our case managers:

  • Educate Individuals about the waiver program, giving them all information and resources needed to make service decisions.
  • Facilitate the Person-Centered Planning process to create individual support and service plans, making sure desires and needs of the Individual are focused on.
  • Help Individuals access non-waiver services such as medical, social, education, and community support.
  • Advocate for the Individual when facing any challenges to help achieve their desired goals and outcomes.
  • Conduct meetings at least quarterly, at least two being home visits, to discuss the Individual’s well-being and satisfaction with provided services.
  • Monitor the quality of services for the Individual and work with providers to resolve any issues.
  • Ensure all required processes and documentation are completed for continued waiver eligibility.
  • Collaborate with supported Individuals to create Annual Planning Documents using the Person-Centered Planning and LifeCourse Framework Process, reviewing documents with each Individual at least quarterly, and with their Individualized Support Team (IST) at least twice a year.
  • Develop and submit initial, annual, and updated Service Plans to outline the Individual’s required services and associated costs to the State.
  • Conduct quarterly Monitoring Checklists with Individuals to ensure services are delivered appropriately, monitor health and welfare, and assess service satisfaction.
  • Perform annual health and safety assessments, ensuring plans are written to address any identified risk factors.
  • Strengthen informal supports and community connections to meet Individual needs.
  • Monitor services from other waiver providers and ensure compliance with planning documents.
  • Send all required documents, including Notices of Actions, to Individuals and their families/guardians.
  • Complete, maintain, and ensure proper documentation, including writing timely case notes, submitting and following up on incident reports, ensuring that contact information for the individual and team is current in all State systems, and keeping files in accordance with State standards.

Wraparound Facilitation

Wraparound is a voluntary program designed to support families with children ages 6-17 facing emotional and/or behavioral challenges. With groups made of caring professionals, friends, and family, our Wraparound teams utilize proven principles to meet each family’s unique needs and help children thrive.

Participants in Indiana’s Wraparound Program receive various tailored services such as family support training, habilitation, and respite, all aimed at helping them achieve their family vision. These strengths-based services respect individual beliefs and customs, focusing on meaningful outcomes outlined in the Plan of Care (POC).

Our dedicated IPMG Wraparound Facilitators make sure participants have the tools and support necessary to integrate into their communities, build supportive relationships, make more positive behavior and lifestyle choices, and improve family communication skills.
