IPMG provides Case Management services for Individuals with developmental and/or intellectual disabilities who participate in Indiana’s Home and Community Based Family Supports Waiver (FSW) or Community Integration and Habilitation Waiver (CIH). Apply for these waivers through your local Bureau of Disabilities Services (BDS) field office.
IPMG provides Care Management for Hoosiers receiving the Health and Wellness Waiver and Traumatic Brain Injury Waiver (TBI). Amendments to Indiana’s Medicaid Waiver program went into effect on July 1, 2024 for the new PathWays for Aging and Health and Wellness (H&W) waivers – formerly combined in the Aged and Disabled (A&D) waiver. Amendments to the Community Integration and Habilitation (CIH), Family Supports (FSW), and Traumatic Brain Injury (TBI) waivers also went into effect.
Effective July 1, individuals age 59 and younger who had been receiving services through the Aged and Disabled waiver will receive services through the Health and Wellness waiver. IPMG provides care management for Hoosiers receiving the Health and Wellness Waiver. Information and resources for individuals transitioning from the Aged and Disabled Waiver to the Health and Wellness Waiver can be found on FSSA’s website.
IPMG provides Wraparound Facilitation services for children with emotional and/or behavioral challenges through the Child Mental Health Wraparound (CMHW) program. Contact IPMG at (765) 464-2991 for application information.
The purpose of these programs is to provide individuals who might otherwise be faced with institutionalization with access to community-based services and supports that are important to them and that are provided in a manner that respects their personal beliefs and customs. If you do not currently receive waiver services, you may apply for them through the corresponding methods listed above.
IPMG provides services statewide, in every county in Indiana. Our Case Managers, Care Managers and Wraparound Facilitators live in or near your community and are therefore able to link you to valuable local resources and to assist you with becoming a valued member of that community. If you would like to work with IPMG, you need only notify your local BDDS office or your local Access Site (for Wraparound) and a referral will be sent to us letting us know of your choice. We accept all referrals and do not discriminate in any way among those who have chosen us. We further ensure that our services are accessible to everyone. We will provide American Sign Language (ASL) interpreters as needed and materials that are appropriate for Individuals who have visual impairments. We can also provide Spanish-speaking interpreters.
